Donation forms
Custom questions on forms
Ask donors something extra, such as T-shirt size, church membership or how they heard about you, and see the answers on each gift.

The Questions tab in the form builder lets you add up to ten questions per form.
Question types
- Short answer ("Who referred you?")
- Paragraph (a longer note)
- Dropdown with your own options, at least two ("How did you hear about us?")
- Checkbox ("I'd like to volunteer")
Each question can have a short hint under it and can be marked required.
Where answers go
Answers are saved on the gift and shown on the gift page under the donor's details. Because they belong to the gift, the same donor can answer differently each time they give.
Answers are not printed on receipts, and they aren't copied onto the donor record automatically. If you want a permanent, filterable value on the donor (membership status, preferred contact method), add a custom field under Settings → Tags & fields and fill it in from the answer; see Tags, custom fields and households.
Tips
- Keep it to one or two questions; every extra field costs a few donors.
- For "how did you hear about us", a dropdown gives clean data; free text doesn't.
- If you already know the source (a specific email, a flyer), make a dedicated form or campaign for it instead of asking.
